Music Therapy in Down Syndrome: A Scoping Review of Conceptual and Methodologial Inconsistencies

Abstract

Music therapy has been increasingly reported in the literature involving individuals with Down syndrome. However, its conceptualization, professional implementation, and reporting practices vary widely. This scoping review followed PRISMA-ScR guidelines to map how interventions self-identified as music therapy in individuals with Down syndrome have been conceptualized and reported in the scientific literature. Thirteen electronic databases were searched without restrictions on publication year or language. Studies were included if the intervention was self-identified as music therapy. Seven studies met the inclusion criteria. The included studies were examined with regard to conceptual consistency, intervention characteristics, professional qualifications of the professionals who implemented the intervention, and reported developmental outcomes. Reported outcomes spanned socio-emotional, cognitive, language, and psychomotor domains, with substantial heterogeneity in intervention formats, study designs, outcome measures, and reporting practices. Only three studies explicitly reported the involvement of a credentialed music therapist and described core therapeutic elements. Although reported outcomes were positive across domains, conceptual imprecision and methodological heterogeneity limit generalization. Inconsistent reporting of professional qualifications and therapeutic processes emerged as a central finding, revealing persistent ambiguities in how music therapy has been operationalized in this field.
